Tourist Visa requirements:

1. Passport
• Valid and old passport (if you have travelled to Japan, Canada, France, Germany, Italy, United Kingdom, United States of America)
• Must be valid more than 6 months from the departure date
• Passport must be signed and must have at least two (2) blank pages

2. Application Form
• Application form should be filled out all items correctly, if item is not applicable, please fill in (N/A). Embassy may not accept application with blank item, no signature or no date in the Form; may deny the application that is filled out incorrectly or has wrong information.

3. Photo
• Specs: 4.5 cm x 3.5 cm (white background, no accessories)
• Must be taken within 6 months
• Please write the applicant’s name and birth on back side photo
• Photo must be pasted on application form

4. Birth Certificate ORIGINAL
• Must be issued within one (1) year from PSA
• If birth certificate is unreadable or has incomplete information, please also provide birth certificate issued by Local Civil Registrar.
• if birth certificate is “Late Registration”, please submit in addition, a Baptismal Certificate and School Record (Form 137) from high school or elementary and School Yearbook (if possible)

5. Marriage Certificate (if married) ORIGINAL
• Must be issued within one (1) year from PSA
• If there no record in PSA, please submit a “Certificate of Non-record” from PSA together with a marriage certificate from the Local Civil Registrar
• If marriage certificate is unreadable or has incomplete, please also provide marriage certificate issued by the Local Civil Registrar.

APPLICANT ARE EXEMPTED FROM SUBMITTING (4) and (5) IF THEY CAN PROVIDE OLD/VALID PASSPORT WITH USED JAPAN VISA. (Unless required as proof of relationship and No change of name)

6. Personal Bank Certificate ORIGINAL
• Must include account type, current balance, account opening date, six (6) months Average Daily Balance

7. Bank Statement (If No Average Daily Balance on Bank Certificate)
• Provide original or certified true copy of bank statements/passbook for the last six (6) months

8. Itinerary (Daily Schedule in Japan)
IF EMPLOYED; - Certificate of Employment ORIGINAL
• Must include applicant's position, date hired, compensation - Income Tax Return Form 2316 (latest)
• Must have signature of the Employer and Employee (Photocopy)

IF BUSINESSMAN/SEL-EMPLOYED;
• Business Registration from SEC or DTI (Photocopy)
• Business or Mayor’s Permit (Photocopy)
• Income Tax Return (Must provide proof of actual payment of tax)

IF SENIOR CITIZEN/RETIREE;
• Senior ID (Photocopy)

IF STUDENT;
• Photocopy of School ID or Original Certificate of Enrollment

FOR GUARANTOR
- Letter of Guarantee
- Proof of relationship between applicant and guarantor (e.g Birth or Marriage Certificate, Photos)
- Bank Certificate [ see (6 & 7) above]
- Tax Payment Certificate (Photocopy)
- Certificate of Employment ORIGINAL Business Documents (Photocopy)

NOTICE:
Size of document for application should be A4 size only. If document is other size, please submit copy that is already adjusted in A4 size from its original and without staples or pasting pages.

1. All Visa fees are non-refundable and non-transferable once paid regardless of the Cancellation/Granting/Withdrawal/Termination/Denial of Application.
2. Amkor Travel and Tours, Inc. does not disclose any results of the visa application/s via phone, SMS, and or email.
3. The embassy does not return any original documents submitted by the applicant/travel agency, except for passports.
4. Amkor Travel and Tours, Inc is not liable for any damages including loss of passports and/or documents during shipment (e.g. grab/lalamove/etc.)
5. Please bring the Original Receipt and a valid ID upon claiming. If passports will be claimed by an authorized representative, please present the following:
• Original Receipt
• Applicant's ID
• Authorization letter
