LIST OF REQUIREMENTS
Applicants Primary Requirements

• Application form: Fully completed in English, signed and dated by the applicant. If the applicant is under the age of 18 both parents must sign the form.
• Two passport size colored photographs
  • Frontal view of the head, recent, lifelike.
  • In color with a uniform and white background.
  • Clear facial features and eyes – not covered by hair
  
• Valid current and old Passport - with photocopy of bio page, stamps/visa
• Travel Insurance Copy of Overseas Medical Insurance policy + Repatriation, evacuation cover (minimum coverage equivalent to €30,000) valid for all Schengen countries.
• Confirmed Hotel and Airline Booking
• Detailed day to day itinerary

IF THE APPLICANT IS AN EMPLOYEE/ BUSINESSMAN

• Original Employment Certificate stating purpose of travel, position, monthly salary and begin date of employment, and approved leave of absence
  • Photocopy of Latest Income Tax Return
• If self-employed/Business, SEC/DTI with Business Permit/Mayor’s Permit
  • Photocopy of Latest Income Tax Return

IF THE APPLICANT IS A STUDENT

• School ID
• School Certificate
• Affidavit of suppor
• PSA Birth Certificate
• Picture together/ Proof of Relationship
• Marriage Certificate (if married)

WITH SPONSOR OR GUARANTOR

• Guarantee Letter
• Personal Bank Certificate with Bank Statement
• If Employed: Certificate of Employment and Income Tax Return
• If Self-employed: Business Permit, SEC or DTI, Income Tax Return
• Proof of relationship (like Birth and Marriage Certificate or Pictures Together).

TERMS AND CONDITIONS

• All Visa fees are non-refundable and non-transferable once paid regardless of the Cancellation/Granting/Withdrawal/Termination/Denial of Application.
• Amkor Travel and Tours, Inc. does not disclose any results of the visa application/s via phone, SMS, and or email.
• The embassy does not return any original documents submitted by the applicant/travel agency, except for passports.
• Amkor Travel and Tours, Inc is not liable for any damages including loss of passports and/or documents during shipment (e.g. grab/lalamove/etc.)
• Please bring the Original Receipt and a valid ID upon claiming. If passports will be claimed by an authorized representative, please present the following:
  • Original Receipt
  • Applicant's ID
  • Authorization letter
