Tourist Visa requirements:
1. Application form
• Fully completed in English, signed and dated by the applicant. If the applicant is under the age of 18, both parents must sign the form.
2. Two passport size-colored photographs
Requirement:
• Frontal view of the head, recent, lifelike.
• In color with a uniform and white background.
• Clear facial features and eyes – not covered by hair.
3. Valid current and old Passport
• Photocopy of bio page, stamps/visa.
4. Documents to show you are a visitor
For Employee:
• Original Employment Certificate stating purpose of travel, position, monthly salary and begin date of employment, and approved leave of absence.
• Photocopy of Latest Income Tax Return.
For Self-employed:
• Photocopy of Business permits.
• Photocopy of DTI or Sec Registration.
• Photocopy of Latest Income Tax Return.
For Students:
• Original Certificate of Enrollment with dry seal.
• Photocopy of School ID – front and back.
For Retiree/Senior Citizen:
• Photocopy of Senior ID.
• Proof of retirement.
5. Original Bank Certificate and Bank Statements for the last three months.
6. Confirmed Hotel and Airline Booking.
7. Detailed day to day itinerary.
8. PSA Birth Certificate.
9. PSA Marriage Certificate
• If married.
10. Affidavit of Support
• If supported by another person.
Terms & Conditions:
1. All Visa fees are non-refundable and non-transferable once paid regardless of the Cancellation/Granting/Withdrawal/Termination/Denial of Application.
2. Amkor Travel and Tours, Inc. does not disclose any results of the visa application/s via phone, SMS, and or email.
3. The embassy does not return any original documents submitted by the applicant/travel agency, except for passports.
4. Amkor Travel and Tours, Inc is not liable for any damages including loss of passports and/or documents during shipment (e.g. grab/lalamove/etc.)
5. Please bring the Original Receipt and a valid ID upon claiming. If passports will be claimed by an authorized representative, please present the ff:
• Original Receipt
• Applicants ID
• Authorization letter
6. Strictly No Cancellation/Passport pull-out allowed once lodge at the Embassy.
